Important Notes

If there are no issues that need to be resolved, you should be exited within 2-3 weeks. Any errors found in your Monthly Reports or other service requirements must be addressed before UHEAN Staff can exit you.

If there are errors and corrections need to be made the exit process will take longer, so please check your email often. You have 30 days from your last day of service to exit the program, so do not delay this process!

It is your responsibility to complete all the exit items included on this page. The Utah Higher Education AmeriCorps Network cannot exit you with award until all items are done.

Exiting the Program

Members can be exited anytime but should pay special attention to December, April, and May months when many other members also want to exit. This is especially important for members who are graduating.

If you are within 2 weeks of exiting, please continue. You can check how many hours you have left on your dashboard in IPT.

What you will need:

  • If you are Exiting Early, your Compelling Personal Circumstance Letter as explained by your Campus Coordinator.

Complete the Exit Process Steps as Follows

1. Do a Self-Audit, please make sure that your monthly Service Hour Record and Monthly Progress Reports are completed correctly. If you have questions about if they are completed correctly, look at the examples or watch the how-to video. Make sure the information is accurate, these are legal documents. Your Campus Coordinator can unlock your forms if changes need to be made.

2. Complete the Unlock Exit Form Request. Found Here

3. UHEAN Staff will add you to the queue for exiting members and unlock (2) forms.

4. When you see the “Exit Form” is available, it can be completed immediately. Your Site Supervisor will need to complete and sign the “End of Term Evaluation” before you can sign it. The Site Supervisor will receive a notice in their IPT Account that your “End of Term Evaluation” is available to complete and sign. Make sure the information is accurate, these are legal documents.

5. Your Site Supervisor and Campus Coordinator will sign off on the forms with their signatures after any corrections need to be made.

6. UHEAN will complete a final audit and ensure that all of your forms are correct and in compliance, we will contact you if anything needs to be addressed.

7. When your IPT reports and Exit forms are completed and signed off you will receive an email notification, from UHEAN, informing you to login into your myamericorps account to complete and sign a Certification Service Form. Once this form has been completed and signed, UHEAN will then successfully exit you from the program.
